Blood: Why Your Panel Tells a Story
Why blood testing matters, how Portico reads physiological targets, and why repeated draws reveal direction.
Why test blood at all?
You can feel tired, hungry, stressed, or strong. But you cannot feel ApoB particles entering an artery wall. You cannot feel insulin rising year by year while glucose still looks normal. You cannot feel low-grade inflammation until it has already shaped the system around it.
Blood is the highway system of the body. It carries fuel, hormones, immune signals, fats, proteins, and waste from one place to another, so checking the highway tells you how the broader system is moving.
That is why blood matters. It turns hidden physiology into something measurable. Not everything, not perfectly, and not forever, but enough to give Portico an anchor for the story your daily signals are telling.
There are three reasons blood work earns a foundational place in Portico.
It shows the quiet systems. Blood can reveal metabolic pressure, cardiovascular particle burden, inflammation, liver stress, hormones, and nutritional reserves before you feel anything.
It gives wearables an anchor. Sleep, heart rate, meals, and training show behavior and response. Blood shows whether the underlying physiology is moving with them.
It turns change into evidence. One result is useful. A repeated result shows whether daily choices are improving the internal system or letting it drift.
Reference ranges are useful. They are not the whole story.
Lab reference ranges are usually built from the population a lab sees. They help answer one question: is this value unusual compared with that population?
Portico asks a different question: what does this value tend to mean inside the body, and how does it fit with the rest of your cardiovascular, metabolic, inflammatory, and hormonal picture?
Take liver enzymes as an example.

ALT and AST are enzymes liver cells use internally. When liver cells are stressed or inflamed, more of those enzymes can leak into the blood. A population range can drift upward as the population mean drifts upward; Portico's target stays closer to the physiological question: how quiet should this signal be when the liver is not under pressure?
Four small numbers carry unusually high signal.
There are thousands of useful biomarkers, especially when you are investigating one specific organ, pathway, or clinical question. For a whole-body read, a few markers behave more like dashboard gauges: not the entire engine, but enough signal to know whether cardiovascular, metabolic, and inflammatory systems deserve a closer look.
ApoB counts the atherogenic particles moving through the bloodstream. LDL cholesterol measures cargo. ApoB counts vehicles.
Fasting insulin can move before fasting glucose or HbA1c. It shows how hard the body is working to keep blood sugar looking normal.
hs-CRP is a broad signal of low-grade inflammation. It does not explain the whole story, but it tells Portico when background noise is rising.
Uric acid is a quiet marker that can rise with insulin resistance, fructose load, oxidative stress, and pressure inside the metabolic system.
One draw shows today. Repeated draws show direction.
A single blood test is a snapshot. Repeating the right markers gives you direction: whether insulin is climbing, inflammation is quieting, ApoB is falling, or liver stress is settling after changes in food, sleep, movement, and recovery.
The danger is rarely one dramatic jump. It is the slow climb you only see when the same marker is repeated.
For example, fasting insulin can drift for years before glucose looks abnormal. A person may have normal fasting glucose while insulin quietly rises from 4 to 10 to 30 to 40 uIU/mL across adulthood. The glucose number can look controlled because the body is working harder and harder to keep it controlled.
That is the point of cadence. Portico is not trying to collect blood for its own sake. It is trying to turn isolated lab values into trajectories.

Blood is not the whole body.
A blood test is still a snapshot in time. Some markers, like HbA1c, integrate roughly across weeks or months, but most markers do not show real-time inflow and outflow. They also do not capture everything happening in tissue, arteries, sleep, meals, stress, or training.
