Exercise: The Only Pill Worth Taking

Why exercise is the most powerful intervention in human health, how aerobic fitness, strength, and balance each protect you, and how Portico turns it into a personalized protocol.

The one pill

If scientists could only ever create a single drug, there is little doubt what it would be.

It would lower resting heart rate. Reduce blood pressure. Improve insulin sensitivity. Decrease visceral fat. Lower ApoB. Reduce systemic inflammation. Protect against type 2 diabetes, cardiovascular disease, stroke, dementia, depression, and several cancers. Extend not just lifespan but healthspan — the years lived with full function. Grow new neurons in the brain. Regulate mood. Sharpen cognition. Improve sleep.

No single drug does any more than one or two of these things. Exercise does all of them.

It is not an exaggeration to call exercise the most powerful intervention in human health. It is the one tool that reaches nearly every system, every risk factor, every organ. We have not found a pill that replicates it. We are unlikely to.

There is a reason for this. For most of human history, exercise was not an intervention — it was just life. Our ancestors walked several miles every day, carried heavy loads, climbed, built, and labored. The human body was shaped by millions of years of that activity. Its cardiovascular system, its muscles, its hormones, its brain — all of it evolved in the context of consistent, varied physical demand. What we call exercise today is simply the attempt to restore what the body was designed to expect. We removed the activity from modern life. The intervention is putting it back.

The paradox of exercise

Here is something worth holding in mind: if you took a snapshot of the body in the middle of hard exercise, almost everything would look alarming.

Blood pressure is elevated. Heart rate is racing. Cortisol — the primary stress hormone — is high. Glucose is being consumed rapidly. Muscles are generating lactic acid. The immune system is in a state of alert. To a clinician reading those numbers cold, it might look like a crisis.

And yet this is one of the most beneficial states the human body can be in — at least temporarily.

Exercise is not a threat. It is a rehearsal. Every time the body is pushed into that activated, stressed, sympathetically driven state — and then allowed to recover — it comes back slightly more capable. Resting heart rate drops. Heart rate variability improves. The heart gets stronger. Cortisol settles lower at rest. The autonomic nervous system becomes better at switching fluidly between the sympathetic (fight-or-flight) and parasympathetic (rest-and-recovery) modes.

That switching ability — the speed and smoothness with which the body moves from activation to recovery — is one of the most important properties of a resilient physiology. Exercise trains it directly, in every session.

Beyond the autonomic system, muscle tissue under load releases a family of signaling proteins called myokines — chemical messengers that travel through the bloodstream and influence fat metabolism, insulin sensitivity, inflammation, and brain function. One of the most studied is BDNF (brain-derived neurotrophic factor), which promotes the growth of new neurons in the hippocampus. The brain literally rebuilds itself in response to physical activity. This is part of why exercise is among the most effective interventions known for cognitive decline and depression.

Aerobic fitness: the triangle

Think of aerobic fitness as a triangle.

The base of the triangle is your aerobic efficiency — specifically, how well your body can burn fat as a sustained fuel. A wide base means you can work for a long time using stored energy, without depleting glucose, without accumulating fatigue. This is what exercise scientists call Zone 2 fitness.

The height of the triangle is your VO₂ max — the maximum volume of oxygen your body can absorb and use per minute, per kilogram of bodyweight. VO₂ max represents your ceiling: the highest aerobic intensity you can sustain. It is the most studied metric in exercise science, and one of the strongest predictors of longevity ever measured.

Maximizing aerobic fitness means maximizing the area of the triangle. A high VO₂ max with a narrow base produces someone who can push hard briefly but fades fast. A wide base with a low ceiling produces endurance without capacity. The goal is both — a wide, efficient aerobic foundation and a high peak. Zone 2: building the base

Zone 2 is the intensity at which your body generates energy primarily from fat, with your heart rate elevated but your breathing still controlled. It is conversational but deliberate — you could speak, but you would choose not to. At this intensity, you are training the mitochondria inside your muscle cells to become more numerous, larger, and more efficient at using fat as fuel.

A wider Zone 2 base means you can sustain effort longer, recover faster between harder sessions, and operate on fat stores rather than glucose during everyday activity. It is the metabolic foundation everything else builds on, and it is largely undertrained in most people.

VO₂ max: the deepest predictor

VO₂ max is not only a performance metric. It is a health metric.

Studies following hundreds of thousands of people across decades consistently show that cardiorespiratory fitness — measured by VO₂ max — is one of the strongest predictors of all-cause mortality, outperforming most traditional risk factors including blood pressure, smoking status, and cholesterol. The mortality benefit of moving from the lowest fitness quintile to the next one up exceeds that of eliminating almost any individual disease risk factor.

VO₂ max declines with age — roughly 1 percent per year beginning in the mid-twenties. The decline is not optional. But its pace is not fixed. With consistent aerobic training, the rate can be slowed substantially. A focused training block can improve VO₂ max by roughly 10 percent in most individuals, regardless of starting point or age.

The table below shows approximate average VO₂ max values by age and sex for healthy, moderately active adults. These are not elite numbers — they represent the middle of the population curve for people who move regularly. Where you sit relative to these norms, and whether your trend is rising or falling, is what matters.

These numbers decline with age. The goal of a sustained aerobic protocol is not to defeat aging — it is to stay as far above these averages as possible, for as long as possible.

Strength: use it or lose it

After the age of thirty, the body begins losing muscle mass at a rate of roughly 1 percent per year. The rate accelerates over time. By the sixties, cumulative loss is significant — not just in appearance, but in function, metabolism, and resilience.

Muscle is not passive tissue. It is metabolically active, consuming glucose, producing myokines, and supporting nearly every physical task the body performs. Losing it quietly over decades is one of the most damaging and least visible aspects of aging.

Muscle is also the body's largest glycogen depot — storing approximately 400 grams of glycogen compared to the liver's 100 grams. This makes muscle the primary buffer for glucose entering the bloodstream after a meal. More muscle means more storage capacity, which means glucose is absorbed and cleared more efficiently, insulin does its job with less resistance, and the overflow that would otherwise be directed toward fat is handled instead by the muscle tank. Building and preserving muscle is one of the most direct interventions for metabolic health, glucose control, and insulin sensitivity available to us.

The intervention is direct: resistance training.

Research consistently shows that people in their sixties, seventies, and even eighties can make meaningful strength gains with structured training. Muscle responds to stimulus at any age. It is never too late to start, and even a modest baseline — full-body resistance training, 30 minutes per session, twice a week — is enough to substantially slow muscle loss and, in many cases, partially reverse it.

Grip strength

One of the most unexpected findings in longevity medicine is that grip strength is a surprisingly powerful predictor of mortality.

The Prospective Urban Rural Epidemiology (PURE) study, following more than 140,000 adults across 17 countries, found that grip strength predicted cardiovascular death and all-cause mortality as strongly as — or more strongly than — systolic blood pressure. Each 5 kg decrease in grip strength was associated with a 17 percent higher risk of cardiovascular mortality.

Grip strength is not a direct cause of longevity. It is a proxy — a signal of overall musculoskeletal health, functional reserve, and the trajectory of physical aging. Portico measures grip strength as one of its core strength markers precisely because it captures so much in a single number.

Balance: the forgotten pillar

Balance is the third pillar — and the most underappreciated.

The ability to maintain stable, controlled posture during movement is a trainable skill. It depends on strength, proprioception (the body's real-time sense of its own position in space), and the coordination of multiple systems acting together. Like all skills, it degrades without practice. Like all skills, it responds to training.

For younger people, balance work builds the proprioceptive foundation that supports everything else — efficient movement, injury prevention, athletic output. For older adults, it is a direct intervention against one of the most consequential risks in later life: falling.

Simple practices — standing on one leg, controlled step-ups and step-downs on a stable box, single-leg loading and unloading movements — train exactly the neuromuscular patterns that protect against falls. None of them require a gym. All of them compound over time.

The cost of a fall

Hip fractures in older adults are far more dangerous than most people realize. Research estimates that between one in four and one in three adults over 65 who suffer a hip fracture die within the following year — from complications, immobility, infection, and the cascade of decline that follows. For the oldest and most frail patients, that figure is higher still.

This is where strength and balance converge. Strength training builds the muscle and reflexes that stop a stumble from becoming a fall. Balance training sharpens the proprioceptive awareness and coordination that keep the body stable in the first place. Together they form the most effective defense against this risk.

Good balance is not incidental to fitness. It is one of its final expressions: the body's ability to control itself in space, under load, in real conditions.
