Looking a little deeper than body weight

Body weight is a useful population-level metric and a simple personal signal, but it does not show where energy is being stored. Visceral fat looks a little deeper. That is why Portico pays attention to it.

Carbohydrates, protein, and fat

Before we get there, it helps to zoom out and talk about the three big macronutrients your body uses every day, and how your overall energy system works: carbohydrates, fat, and protein.

Carbohydrates are the quick energy source. They are easy to break down and useful when your body needs fuel now. Protein is mostly structural. It helps build and repair muscle, enzymes, hormones, and tissues. Fat is the deep energy reserve. It is dense, efficient, and built for storage.

That density matters. Carbohydrate provides about 4 kcal per gram. Protein also provides about 4 kcal per gram. Fat provides about 9 kcal per gram, which is more than twice as much energy in the same weight.

From a biology point of view, fat is an extraordinary storage molecule. The body needs a way to store energy safely, compactly, and for long periods of time. Fat cells are that storage system. Fat enables a deterministic source of energy for the human body and is one of the reasons we were able to develop such a large and intelligent brain.

Fat is not the problem. Location is.

Most stored fat sits under the skin. This is called subcutaneous fat.

Subcutaneous fat is not automatically harmless, but it is the body's intended storage compartment. Think of it like a bathtub. The tub is designed to hold water. As long as the water stays inside the tub, the system is mostly doing its job.

The problem begins when the storage system starts to overflow.

When the body has more energy to store than the safer storage compartment can comfortably handle, fat can begin accumulating deeper inside the abdominal viscera, around the organs. This is visceral fat.

Visceral fat is not just "extra fat." It is fat in the wrong place.

Why visceral fat matters

Visceral fat sits in the viscera, around organs such as the liver, pancreas, and intestines. It is more metabolically active than subcutaneous fat and is strongly linked with insulin resistance, inflammation, abnormal cholesterol patterns, fatty liver, and higher cardiometabolic risk.

That is why two people with the same body weight, or even the same BMI, can have very different metabolic risk. One may store more energy safely under the skin. Another may store more of it around the organs.

This is also why Portico does not stop at weight.

Weight can tell you how much mass changed. BMI can give a rough starting point. Estimated body fat can add more context. But visceral fat asks a deeper question:

Where is the fat stored?

That question matters because visceral fat is connected to the metabolic pathways that sit upstream of many long-term health risks, including cardiovascular disease, type 2 diabetes, fatty liver disease, and possibly risk patterns involved in cancer and neurodegenerative disease.

No single number tells the whole story. But visceral fat is one of the numbers worth knowing, and Portico treats it as one of the three key metrics for the Sage domain.

How to measure visceral fat

There are levels of precision.

1. Imaging: the gold standard

The most direct ways to measure visceral fat are imaging methods such as CT and MRI. They can see the fat compartments inside the abdomen with high precision.

In everyday health tracking, DEXA is often the more practical benchmark. A DEXA scan is a quick, low-dose X-ray scan that measures bone, lean mass, fat mass, and fat distribution. Many modern DEXA reports include an estimate of visceral fat.

For most people, a DEXA scan once a year is a reasonable benchmark. It gives Portico a higher-confidence body-composition snapshot and helps calibrate the signal you can collect more often.

2. Smart scale: the frequent signal

Between infrequent DEXA scans, a smart scale can be a practical alternative for frequent tracking. It uses bioelectrical impedance to estimate body composition. It is not as accurate as DEXA, but it has one powerful advantage: frequency.

If the same individual steps on the same scale under similar conditions, ideally at the same time of day, the trend can become useful. Portico can use that repeated signal, with DEXA as additional calibration, to better understand direction and momentum.

Daily or weekly signals do not need to be perfect to be useful. They need to be consistent enough to show movement.

3. Waist: the accessible starting point

Short of imaging or a smart scale, waist circumference is the simplest practical signal.

It is not perfect. It cannot see inside the abdomen. But waist captures central body size, and central body size is closely related to visceral fat risk.

That is why Portico starts there.

A single waist measurement helps Portico estimate your visceral fat baseline. Over time, that baseline can improve with better inputs: smart scale data, DEXA, blood markers, activity, sleep, glucose, and other signals.

Portico's goal is not to worship one number.

The goal is to keep upgrading the signal.
